We ask potential clients to read through our information, then complete a
referral form with someone who knows them well acting as a referee e.g. GP, Care Co-ordinator.
A risk assessment will need to be completed by potential clients and their referee.
Referrals are generally made by car co-ordinators. (e.g. social
workers and community mental health nurses) but can be accepted
from a veriety of other sources, such as another professional (e.g.
G.P, psychiatrist or psychologist), a carer or the person experiencing
mental health difficulties themselves.
On receipt of a referral form we will arrange for the potential client to
attend a meeting with 2 of our workers in order to complete an
assessment, where we will aim to gain a better understanding of
someone's needs. (This will include an assessment of risk). We will
aim to see potential clients within two weeks.
The assessment meeting is a 2-way process where we aim to
determine if our project is best suited to someone's needs. The
meeting also gives the opportunity for a potential client to find out
about us and to decide if we are the right choice for them.
There is usually a short waiting time for our supported
accommodation. We keep people on the waiting list regularly
informed of any progress in allocating support.
